NCEPOD are exploring the process of the transition of young people with complex chronic conditions from child to adult health services.
The aim of the study is to explore the barriers and facilitators in the process of the transition of young people with complex chronic conditions from child to adult health services.
Please complete this survey if you are or could be involved with this group of young people. NCEPOD are hoping to collect data from health and social care providers across a range settings, including primary care, community care (including school nursing and hospices), physical healthcare, mental healthcare, commissioning and social care.
